Default why does formatting apply to the entire document?

I just upgraded to Word (and Office) 2003 and this is driving me crazy. When
I choose any type of formatting, it gets applied to the entire document
rather than just the text I have selected. This happens when I have not
selected any text at all, and when I have selected an entire paragraph. I
can usually avoid the problem if I select a small portion of text (without
the paragraph marker) before choosing the formatting.

For example, if I type a list, then select the item (paragraph) in the list
and click "underline" OR press CONTROL-U, then the entire document becomes
underlined. Or, if I type a phrase, press return, and then click the
"Bullets" button in the tool bar, every paragraph in my document becomes
bulleted.

If I undo immediately, then the document looks the way I intended. From
this, I guessed that it had something to do with auto-formatting, and turned
of every auto formatting feature I could find, but that didn't work.

I also searched various ways and read this article on support.microsoft.com,
but it doesn't address the issue, nor does the fix they suggest apply.
