Default Losing formatting whenever close a word document

When I work on a Word document it will look fine. I save it but when reopen
all formatting effects have been lost but all the formatting commands are
listed below the unformatted text,
Any ideas?

In Word, click on File | Open. In the "Files of type" box, is "Recover
text from any file" selected? If so, change it to "Word documents",
"All Word documents", or "All files", choose your file, and click on
Open.
