Default Checkbox List but also allows user to type

How do I create a form that has checkboxes in one section and also allows the
user to type responses in another section?

If there isn't already one, insert a section break between the two sections.
Then go to Tools Protect Document, click Forms, and then click the
Sections button next to it. In the next dialog, uncheck the section(s) that
should be editable. Click the OK button in each dialog, and save.
