Default Header

I have a document that the previous person set up several headers. I delete
them all, but when I put my header in for the "whole document" the header is
only on the first 4 pages and the document is 200 pages.

Does the document contain section breaks? If so, the headers are unlinked.

I don't know if they are linked. How can I tell?

On page 1, View | Header and Footer. On the Header and Footer toolbar, click
Show Next to go to the header/footer for Section 2. If the Same as
Previous/Link to Previous button is turned off (dimmed, pushed out), click
on it to turn it on.
