Default Would like to apply "read only" NOT "recommended"

I need to know how to protect a document as a "read only" - but not using the
"read only recommended" aspect on Word 2003

If the purpose is to prevent others from changing your document, there's
nothing you can do to prevent it completely. If it's just so they don't mess
up your document, send it as a .pdf file.

This would be used for an in-house template. The idea is to use the template
and modify as necessary, but being forced to save it as a new document so as
to save the template in original form. The .pdf version would not work in
this scenario.

You can right click the document and go to properties and click the read-only
there, but it is just as easy for someone to unclick it. This will work for
what you want it to though

To do this you create a true template (.dot file), which users select from
the New (or Templates) dialog. The default action for .dot files is New
rather than Open, so the template is never touched. You can make the
template read-only if desired, but it shouldn't be necessary. See
