Default How do I insert a checkbox in a form in Word 2007?

Word 2007 no longer has the Forms toolbar and I cannot figure out how to
insert a checkbox in a form!

First click the Office button, choose Word Options at the bottom, and check
the box for "Show the Developer tab on the Ribbon."

On the Developer tab, in the Controls group, click the Legacy Controls
button. You'll find the former Forms toolbar there, along with the ActiveX
controls that used to be on the Control Toolbox. The Protect Document button
is at or near the right end of the Developer tab.
