Default Missing auto-text entries in headers and footer drop down

When trying to insert a header or footer, my auto-text drop-down is a tiny,
empty box. No selections to choose from. I am completely baffled. Any
ideas? A re-install did not work.

The usual reason for this is that a document or template has been saved as
Normal.dot.
