what is the correct layout for a bibliography?

The bible for these types of issues is "The Chicago Manual of Style". I have
the 15th Edition, 2003, ISBN 0-226-10403-6

It depends on the discipline/field/topic and what your professor
wants--e.g., Chicago style, APA style, MLA style. This site gives several
examples, or google for a template.
