What you are doing wrong is creating an index instead of a table of
contents, which is what you apparently want. See http://www.ShaunaKelly.com/word/toc/CreateATOC.html -- Suzanne S. Is it possible you are confusing the terms "index" and "table of contents?"
The index appears at the end of the document, and contains specific terms sorted in alphabetical order. The table of contents (TOC) appears at the front of the document and typically lists the chapters and one or more levels of heading in the same order as they appear within the body of the document.
