There are 3, 3rd level headings between figure 12 and 13. My Table number
had also started to do the same thing. Any ideas?

Another thing to consider: If you are using track changes, numbering
can be incorrect, until you've accepted (or rejected) changes.

The Figure captions were being number correctly at Figure 4.12 but

inserted the caption for figure 4.13, the caption dialogue box

Figure number to be 4.1. When I clicked ok, what was entered was
Figure 0.1.
The chapter number was reset to 0 and the figure number was reset
to 1 right
in the middle of the chapter for no apparent reason. I was not able
to reset
caption number to display Figure 4.13.

How do I re-establish the correct sequence for both the chapter
number and
figure number?
