Not sure what is occuring here, but one place to check to see if anything has
inadvertently happened:
Customise Toolbars, Commands, Keyboard, Categories - View, Commands
-ViewOutline, the Current Keys (ie shortkey) should be Alt+Ctrl+O.
If it is anything else, that could be the problem.

Maybe someone else may know of a bug or other issue that can affect this.
