The TOC will pick up only the number defined by the PAGE field; anything
outside the field will not be picked up. To add the appendix number to your
page number in a way that will cause the TOC to pick it up, you have to use
the "Include chapter number" setting in the Page Number dialog, and that
requires that the "chapter number" be the number of an autonumbered style
which must be one of Word's built-in heading styles. For more, see

My problem is that the Table of Contents does not display the proper
format
for the Annex page. It does not display F-x/y. It simply show "x"
How can I fix this?
