Float repeating object top of each page?
Select the "Different first page" option (in the Page Setup dialog box),
bookmark the table on the first page, and then insert the REF field in the
main header.

While the people using the form are very bright, they are not exactly great
with Word, so using the DocProperties, Style Refs, macros, updating, etc. is
going to be out of the question. Isn't there some way to lock and repeat a
table or a bookmark reference throughout a document?
