The particular symptoms you have encountered are unusual, though. :-(

I have solved the issue though! It turns out that the missing captions
were listed in the dialog box, just out of numerical order (it ran 19,
20, 21, 29, 30, 22, 23...). No idea how that happened... and it's still
doing it after restarting Word/the PC!
