Default caption problem

I know how to do that. This problem just comes up automatically. I can keep
fixing it every time, but it just keeps coming back. I think it happens when
the page regenerates. Any other suggestions? It has to do with 'styleref'

Choose Insert | Reference | Caption. Click the Numbering button. At
"Include chapter number," select "Heading 1," and then click OK. Click
Close to close the Caption dialog box.

The captions for my tables and figures show three digits. (i.e.

Figure
4-2-3) I don't know how to get rid of the middle digit. It keep

popping up
for some reason. The first digit is the section number, the section

digit is
the paragraph number or something, and the third digit is the page

number.
