I can no longer edit any form/templates. I can see the Legacy controls, but

Although the document is unrestricted, I cannot edit in Word 2003 (.dot) or
Word 2007 (.dotx). I've tried making a new document from a copy of the
form/template but no luck.

I can still fill in the form; I just can't edit it. Any suggestiosn?

On the Developer tab of the ribbon, click the Protect Document button to open
the task pane. There should be a button at the bottom for "Stop Protection".

An alternative is to go to Office button Word Options Customize, select the
All Commands category, and add the Lock icon to the Quick Access Toolbar.
