I too am facing a similar problem.
In my document, few headings are being repeated in the TOC. Before the
heading, there is a manual page break, but in the TOC, there is no blank
entry before the main entry - the entry is just getting repeated twice.
I have another exactly same document where no duplication occurs.
Please help!!
