Default Creating a form in word that can be fill in on line

If you are trying to create a *Word* form, see
http://word.mvps.org/faqs/tblsfldsfms/LinesInForms.htm (including the linked
articles). Note that if you are using Word 2007, you'll find the commands
for inserting fields on the Developer tab of the ribbon (click the Legacy
Tools button).
