Separate tools. The simplest thing to do, if you don't want your question
for the user appearing in your final document, is to put the question as the
default text in the form field.

What you are talking about is what Word calls an "online form." Check this
in help. For more about online forms, follow the links at

Chapman's series of articles. You may also want to look at

The pop-up box may be a Fill-In field, which is a separate kind of tool and
usually not appropriate (very many of them and you will really annoy users).
They are not compatible with protected forms.
