Default Header/Footer Toolbox

Assuming you're referring to the Header and Footer toolbar in Word 2003 or
earlier (since I don't think any issue with the Header & Footer Tools Design
tab in Word 2007), it *should* be displayed by default when the header or
footer pane is in focus. By default, it is floating, but it is easily docked
(and I actually prefer it so), so make sure that you haven't docked it
somewhere.

If it is truly gone, you can do the following to bring it back:

1. View | Header & Footer.

2. Press Alt+F11 to open the Visual Basic Editor.

3. Press Ctrl+G to open (or switch to) the Immediate Window.

4. Type:
commandbars("Header and Footer").enabled = true

5. Press Enter.

6. Press Alt+F4 to close the VBE.
