Default TOC in Word 2002

The Header style is intended for use in the page header; it is a Body Text
level style, so it won't be automatically included. You can change the
outline level to Level 1, Level 2, or whatever, but it would be more
straightforward just to use the built-in heading styles (Heading 1, Heading
2, etc.) instead. Note also that text in the page header, regardless of
style, is never included in the TOC.
