It is always better to start Outlook merges from Outlook (and not only with
Word 2003) - http://www.gmayor.com/mailmerge_from_outlook.htm , but I think
your original reply may provide an immediate fix.

Apparently for Word 2003 you should do the mail merge starting in
Outlook.
