Default How to restructure TOC

I'm not clear what you want here, but it looks pretty much like what Word
provides by default. If headings are numbered in the text, the numbers will
be included in the TOC, and you can arrange the alignment of the numbers and
headings by modifying the format of the TOC styles. As for the "continued"
heading, you accomplish that by putting the TOC in a separate section with
"Different first page" enabled and the "continued" heading in the page
Header. The "Sec/Para" and "Page" headings can also be in the header, though
I don't think they're really required, since most people know how TOCs work.
