If you have any Hidden text in the document (TC or XE fields, for example),
be sure that Hidden text is not displayed before generating your TOC (and
index); also make sure that "Hidden text" isn't checked on the Print tab of
Tools | Options. Also, if your document contains tracked changes or
comments, make sure you're printing "Document" and not "Document showing
markup."
