Default I CANNOT make TOC Styles Behave!!

I thought I understood the concept behind using TOC styles, but
evidently not. My Table of Contents has "duplicate" outlined
numbering, ie:

I. I. Heading 1.
A. A. Heading 2.

It looks like the first outlined number is outside of the TOC field
because it doesn't show up in grey when I select the Table of
Contents.

Does anyone have an idea as to what would cause this? I've wasted
two hours trying to fix it.

Thanks!

Jessi
