As G-N-O has indicated, accidentally catching the Insert key is the most
likely answer for that particular problem, but Word 2003 in particular does have a nasty habit of randomly changing certain preferences in the tools options dialog. The only answer is to this is to record the settings in autoopen and autonew macros so at least you start each document with your preferred settings. If you start the macro recorder and switch between the tabs of tools options, you will get the whole set of options recorded. Retain the ones you have problems with and delete the rest. To correct overtype mode - you need the following line present.
