Default Mail Merge Labels

If you have ever saved a header/footer in normal.dot you are stuck with it.
You would have to rename normal.dot to get rid of it. You can check by
starting Word in safe mode (hold the CTRL key whilst starting Word)

And the common reason for this is that you have a header in
normal.dot.
