I'd wager the boxes have not much to do with the TOC per se, but with hyperlinks. Try inserting a hyperlink anywhere else in the file and see if it gets "boxed" as well. ISTR that this might as well be a setting of Reader or wherever you look at the resultion PDF, rather than of the PDF itself.
