You need a nested field like
{ ={ MERGEFIELD COFA:Cost }+{ MERGEFIELD COFA:Fine} } where all the {} are the special field code braces you can enter using ctrl-F9, { MERGEFIELD COFA:Cost } is the mergefield you would use to insert the COFA:Cost value (it may need double quotes around COFA:Cost, for example). Use Alt-F9 to see what's there already.
