Oops! I have used Title and Salutation to convey the same thing. The second
example should have read

Dear {IF {Mergefield Salutation} "" "{Mergefield Salutation} {Mergefield
LastName}"
"Sir/Madam"}

If you have the full information, it is simply a matter of inserting

and
Dear {IF {Mergefield Title} "" "{Mergefield Title} {Mergefield
LastName}" "Sir/Madam"}
All brackets {} are inserted with CTRL+F9 and the fieldnames must be
exactly the names in your data file.
