I just checked several other documents that I created using fields,
and if bookmark is unchecked they are fine. It just did not work on
this doc.

There isn't a "fix" because it isn't a bug -- just a difference in
the option settings.

Every form field is associated with a bookmark that serves as the
field's name. Word is showing you the bookmarks.

Go to Office button Word Options Advanced, scroll down to the
"Show document content" section, and uncheck "Show bookmarks".
