The paper size must be set to the size of the SHEET, not the page after
folding. For example, if you're printing on 8.5 x 11 which will be folded to
5.5 x 8.5, the paper size in the Page Setup dialog must be: Width 11", Height
8.5", and the orientation must be Landscape.
