Avery have a Word template that can be used in conjunction with Avery card
stock.. go to.......

Don't download a special template unless it provides some unique advantage.
(Those from Avery for standard rectangular forms never do provide anything
uniquely helpful.) Use the envelopes and labels wizard and set the option
for the form number you are using.
