Default How do I set up a Form in Word using ___ to type words on

Using Microsoft Word:

I am creating a form that will include boxes to be checked and Lines to be
typed on, how do I insert boxes and lines that do not delete once they are
typed on.

What you are talking about is what Word calls an "online form." Check this
in help. For more about online forms, follow the links at
