I'm afraid I can't help with Help; there is no index in Word 2003. To access
the classic Style dialog, open Tools | Customize, select the Format category
on the Commands tab, scroll down to Style (as opposed to Styles and
Formatting), and drag it to a toolbar.

If you want it on the Format menu so that you can use Alt+O, S to access it,
you will need to remove Styles and Formatting or assign it a different
accelerator key.
