Default Word not in the Right-click New Document contextual menu

When I Right-click in a folder and go to the New submenu I have a selection
of programs to create new files, but there is no choice to make a new Word
document.

How can I add Word to the New document contextual menu?

Also I have something called Microsoft Access Database listed in the New
submenu and that doesn't even work. How do I get rid of that?
