Default How do I limit the document map to use only headings?

Whenever I activate the document map, it comes up with all sorts of text
besides the heading levels. I can manually remove them by changing to outline
level to body text. How do I prevent them from showing up in the first place?

You can cut down on this by disabling "Define styles based on my formatting"
on the AutoFormat As You Type tab of Tools | AutoCorrect Options.

The "Define styles based on my formatting" was disabled, so that solution
will not work.

In that case you have paragraphs other than headings that have a TOC outline
level other than Body Text.
