Default How to use field codes to insert addressee name in pg 2 header?

In a letter template, I want the addressee name to automatically appear in my
2nd (and subsequent) page header, as follows:

Addressee name
Date
Page No.

I have read every entry in the Word help menu, but do not find how to set
this up so it works. Thanks to anyone who can help.

I would use a userform to get the information such as addressee, address,
subject, salutation, etc then have code in the User Form save the data to
document variables and then use { DOCVARIABLE } fields in the template at
the location where you want the information to appear.

Let's not go in to hyperbole here, you have not read every article in help.
You've read every one you could find that you thought would be useful. No
one has read every article in help. Word's help file is very good, but often
you can't find something unless you know how Microsoft called it.

Use a StyleRef field for both your addressee name and the date. (That way if
you change either in your first page, they will be updated automatically.)

Perhaps I wasn't clear. I don't have a style (or as another respondent
suggested, a database of addresses) to reference. I want the 2nd page header
to reference the addressee name that the user is prompted to type in. The
articles I read suggested that I could use the ASK and REF fields with the
bookmark, but I am unable to find instructions that tell me how to set that
up.

Hope this is clearer. Thanks for your quick response. One comment: you
are correct that Microsoft help articles are usually very good, but I am
striking out on this one. It happens. I am not a novice, and I know how to
use search terms.

Well, you need to create a style in order to use StyleRef. For this purpose
it should be a character style. It should be defined as "Default Paragraph
Font +"--that is, it's identical to the underlying text except for the name.
