Can anyone tell me how to make the form field direction to go down to
complete and address and then across and down to complete phone number, fax
number, email address, etc.

Put the form fields in a table. Put all the fields that should be targeted
vertically in the same table cell, pressing Enter for each new line. All the
fields in the one cell will cycle before Word moves to the next cell when TAB
is pressed.
