Default How do I make a form so that previous info doesn't show up when re

I am using Microsoft XP Edition. I am trying to make a form template that
can be used over and over. However, my problem is when I add data to the
form and then send via email to a recipient and they open it, it reveals
previous data that should not be seen. (These forms I use contain
confidential data that should be deleted when the new data is inserted).
