Default Document Map Entries

I have a document that I'm trying to make easier for use.

The document has numerous tables, the first cell in each table is the
'heading' for that information.

HOW do I format the text in the first cell so that it will appear in the
Document Map?

As you've found, text in tables doesn't appear in the DM.
