Default How do I create a template w/prompts for info?

I am trying to create a contract that has fields that prompt the user to put
in information. Something along the lines of the forms generated by Letter
Wizard where the field is deleted when you begin to fill in the information.

(though the Letter Wizard does not, to my knowledge, use MacroButton fields
but instead a UserForm; the fax templates use MacroButton fields).
