Default tick boxes

Hi

I was wondering if anyone can help me. I am trying to add tick boxes. I am
doing it for a school report and there are many boxes in which need to have
ticks added. I need to be able to either click on the box and a tick appears
or have a drop down list where I am able to choose the tick.

Hope someone is able to help. Any help would be much appreciated.
