Default Loss of customized toolbar and autotext entries

A shared document which appears on other pcs with a customised toolbar
containing customised autotext entries appears on my pc without the toolbar.
Occasionally when I first save it the toolbar is intact but after closing and
reopening it has disappeared. Also documents which previously contained this
toolbar on my pc have now lost it.

Toolbars and AutoText are stored in templates; they don't travel with
documents.
