Default Modify index styles

In Word 2003, the Index and TOC styles are not listed in the Styles and
Formatting pane. AFAIK you have to do as if you're creating a new index or
TOC, chose From Template, and modify the styles from there. (Unintuitive to
begin with, but whatever.)

So I can see all the index styles there except Index Heading. How can I
modify that one?

Default Modify index styles

You can always force Word to display All Styles in the Styles dropdown by
pressing Shift while clicking the down arrow. You can also see All Styles
from the Styles & Formatting task pane, choosing Custom, and then clicking
Styles... in the Format Settings dialog.
