Default Docs from Word sent by email with reply fields for completion

I wish to set up Word templates that can be attached to an email which allows
the recipient to answer questions YES \ NO \ See below, and complete text
boxes. The form is then to be returned by email to the sender

Any thoughts would be appreciated

I would think using InfoPath would be a better solution.
