Default Internal documentation template available?

I trying to create a good template for Internal Documentation.

The template(s) should include (at least) sections for the Title of the
document, Category, Date, possibly version, revision, or document number.
The template should be versatile enough include company logo or letterhead.
Also,
template should allow for preformatteded sections allowing the user to
simply fill out the template.

Does such a thing exist?

If you know what you want, why not just create it?

Default Internal documentation template available?

I have. That was actually my first sentence.
But the nature of my question was are there any templates out there set to
achieve the same thing. I'm not opposed to looking at several different
solutions, and I could not find any other templates "out there" that fit this
need.
