Default How can I get check off boxes to work in a word doc sent in html f

I am trying to send a check off survey as a web page so clients don't have to
open and save an attachment to respond. Every time I try it out, the check
boxes and text boxes disappear. How can I get the message to retain all the
formatting?

Since you cannot determine beforehand what email applications your
clients use (they might read their email on a handheld/smartphone/etc.
gadget, or configure their client to show text-only), I would suggest a
different approach. Maybe a PDF-form might be a possibility, or an HTML
form on your webserver?
