Default Insert a picture into the form without unlocking the form?

As part of the completion of a created form I need an end-user to insert a
picture into the form and am not wanting to unlock the form everytime they
need to update the form.

You can download a demonstration template called form_picture.dot at
http://jay-freedman.info.

Please read the instructions in the body of the template and follow them
exactly. I've had a number of questions from people who tried to use it as
the basis for their own templates and somehow couldn't get the bookmarks in
the right places.
