Default Form Automation

I'm feeling very dumb today. I am not a new user however I haven't created a
template for a VERY long time. And cannot, for the life of me, find an
answer.

I have a very simple table 8 columns, 7 rows. In 2 of the columns, of each
row, I'd like to have a drop-down box with #s 1-4 as choices. This form is
completed on a weekly basis.

Could someone kindly help?
